Reporting Guidelines:
Please fill out this form within 72 hours after the completion
of your service initiative and submit it to the Regional Director of Community Service
and your Regional Polaris. One copy must be maintained by the chapter secretary and
another must be sent to the National Director of Community Service. All spaces must
be completed in order to receive credit for the activity. Parties will not be considered
community service unless 100% of proceeds go to support a particular cause or organization.
Additionally, only uncompensated brothers may have their hours counted as part of
the service hours. Guests do not count.
You may be asking….Why should I fill out this
form?
This actually benefits YOU, YOUR CHAPTER, and the FRATERNITY IN WHOLE! In keeping
track of community service data, the Office of Community Service is able to substantiate
nominations for brother and chapter awards on a national level, as well as allow
the Fraternity to position itself properly when establishing new colonies and acquiring
new sponsorship and/or partnerships on every level. Therefore, it’s imperative that
each brother do his part in showing quality and quantitative events that directly
impact the campuses and communities which we serve.
